2023-2024 Registration Forms

REGISTRATION:

Registration information is for families enrolling students in the 2023-2024 school year.

You must submit your medical forms & pay your Education Fee balance IN PERSON during ONE of these sessions at Saint Luke Academy:

• Wednesday July 19th, 8:30am-11:30pm

OR

• Wednesday July 19th, 3:00pm-6:00pm

OR

• ​Wednesday August 9th, 3:00pm-6:00pm

*If you cannot make it to any of these Registration periods, please call the Main Office at (773) 472-3837 to arrange an appointment to submit your paperwork.  We will not be able to accept ANY medical paperwork electronically.

REQUIRED DOCUMENTS TO BE SUBMITTED:

NEW FAMILIES:

• Birth Certificate  - Every NEW family to the Academy must bring an ORIGINAL birth certificate for each student they are enrolling.  We will make a copy for our files and give the original(s) back. If you are an existing family and we do not have a birth certificate on file, we will contact you.

• ​Blackbaud Tuition Management Form - please enroll in our tuition management portal, button below, and choose the September-June plan. The first week of school (August 28-September 1, 2023) will be added into June 2024's monthly payment:

NEW & RETURNING FAMILIES
These forms must be turned in at Registration:​

​-  All students entering Kindergarten and 2nd Grade are required to have a dental exam:

-  All students entering Kindergarten are required to have an eye exam
-  Students entering grades 1st or 2nd for the first time in an Illinois school are also required to have an eye exam:

• Child Health Examination and Immunization Form 

- All students at Saint Luke Academy are required to have a health exam AND up-to-date immunizations on file every year (one per student):
