REQUIRED DOCUMENTS TO BE SUBMITTED:

NEW STUDENTS:

• Birth Certificate  - Every student NEW to the Academy must submit an ORIGINAL birth certificate during Registration.  We will make a copy for our files and give the original back.

                                                     
New Student Playdates
New students can bring in their birth certificates to one of our New Student Playdates on our playground on either:  
​      Friday, June 14 from 10:30 - 11:30am 
                                     or
      Thursday, July 25 from 10:00 - 11:00am

• ​Blackbaud Tuition Management Form - please enroll in our tuition management portal, Blackbaud, and choose the September-June plan to be billed the 1st of each month.

​-  All students entering Kindergarten and 2nd Grade are required to have a dental exam:

-  Students entering grades 1st or 2nd for the first time in an Illinois school are also required to have an eye exam:

- All students at Saint Luke Academy are required to have a health exam AND up-to-date immunizations on file every year (one per student):
